Record Review

With only a few exceptions (as per confidentiality guidelines), you have the right to review your counselling records and see what personal information is held on file about you. In order to review your file, we can schedule a meeting where I can help you understand the information recorded on file (e.g. assessment information, technical jargon etc.). Confirmation of your identity and a signed consent form are required. I reserve the right to charge a nominal fee for such requests.

If I am unable to grant you access to your records, I will advise you of this, and the reason I am unable to comply with your request, within 30 days.

If you believe there is an error in the information I have regarding you, you have the right to ask for a correction. This applies to factual information and not to any professional opinions formed. A brief statement from you regarding your version of the information will be included in your counselling record. Regardless of any changes, additions or deletions made, the original version of the record must legally continue to be accessible.

 

Professional Misconduct

If you are ever unhappy with the services provided, your feedback is appreciated. If for any reason, you believe that your counsellor has acted in an unethical manner, you have a right to provide a formal complaint to either my Supervisor or Regulatory Body.

The procedure for doing so is to first address the issue with the counsellor, for resolution. If this can not be  accomplished, you may address your concerns in writing to the Association of Cooperative Counselling Therapists and/or my Clinical Supervisor.

 

Transfer of Counselling Records

If you are no longer a client of Pink Sky Counselling and you would like your counselling records transferred to  another provider, please send your request in writing and a “Transfer of Counselling Records Form” must be completed. The Counsellor will review the written request and set up a time to review the file with you prior to sending the records. The Counsellor may contact you if further information is needed. A fee is charged for such requests.
